Start by contacting your local County Board of Developmental Disabilities.
They will assign a Service and Support Administrator (SSA) to assess eligibility and guide you through the application process for services like the Individual Options (IO), Level One, or SELF waivers. You can also learn more on DODD’s Waiver Services page.
Ohio DODD offers two main types of transportation:
- • Transportation for accessing waiver services and community activities.
- • Non-Medical Transportation (NMT) for travel to employment, day services, volunteer work, and education programs.
Both services are available under the IO, Level One, and SELF waivers.
The Ohio Individual Service Plan (OhioISP) is a standardized planning tool used statewide to document a person’s goals, services, and supports. It ensures consistency and clarity across providers and counties. If you’re receiving services, your SSA will work with you to develop and update your OhioISP regularly.
Yes, children with developmental disabilities may qualify for early intervention services, family support, and Medicaid waivers. Eligibility is based on age, diagnosis, and functional needs. Contact your County Board for a developmental evaluation.
