Terms & Conditions

Welcome to Zions Caregivers, LLC These Terms & Conditions govern the use of our services provided under the Ohio Department of Developmental Disabilities (DODD). By accessing or using our services, you agree to comply with these terms and all applicable state regulations.

1. Scope of Services

We provide home and community-based services to individuals with developmental disabilities, including but not limited to:

  • Homemaker Personal Care (HPC)
  • Shared Living
  • Supported Living
  • Respite Care
  • Non-Medical Transportation (NMT)
  • Transportation
  • Residential Respite
  • Participant Directed HPC
  • Vocational Habilitation
  • Adult Day Support
  • Congregate Setting

All services are delivered in accordance with the individual’s approved Individual Service Plan (ISP) and Ohio DODD rules.

2. Eligibility & Enrollment

To receive services, individuals must:

  • Be enrolled with Ohio DODD and have an active waiver (e.g., Level One, Individual Options, or SELF)
  • Have an approved ISP outlining authorized services
  • Participate in assessments and planning meetings as required

3. Service Delivery & Documentation

  • Services are provided by trained and certified Direct Support Professionals (DSPs)
  • All service delivery is documented in compliance with Ohio Administrative Code (OAC) 5123 standards
  • Electronic Visit Verification (EVV) is used where applicable

4. Scheduling & Cancellations

  • Services are scheduled based on the ISP and mutual agreement
  • Cancellations must be communicated at least 24 hours in advance
  • Repeated cancellations may impact service continuity and require ISP review

5. Billing & Payment

  • Services are billed through the Ohio Medicaid system or private pay arrangements
  • Any private pay agreements must be documented and signed
  • Clients are not responsible for Medicaid-covered services unless otherwise specified

6. Privacy & Confidentiality

We adhere to HIPAA and Ohio DODD confidentiality standards. All personal, medical, and behavioral information is protected and only shared with authorized parties.

7. Rights & Responsibilities

Clients have the right to:

  • Be treated with dignity and respect
  • Participate in service planning
  • File complaints or grievances without retaliation

Clients are responsible for:

  • Providing accurate information
  • Cooperating with service delivery
  • Maintaining a safe environment for staff

8. Compliance & Oversight

We comply with all applicable Ohio Revised Code and Ohio Administrative Code regulations, including:

  • DSP certification and background checks
  • Incident reporting and MUI (Major Unusual Incident) protocols
  • Annual training and audits

9. Termination of Services

Services may be terminated due to:

  • Non-compliance with ISP or safety concerns
  • Changes in waiver eligibility
  • Mutual agreement or relocation

A transition plan will be developed when appropriate.

10. Amendments

We may update these Terms & Conditions to reflect changes in law, policy, or service structure. Continued use of services implies acceptance of updated terms.
